Our older population and their needs are increasing. We are even more challenged with the unfortunate current state of the world. This can be overwhelming for patients, families and our communities. We at Chanda Care hope to contribute and can both help our community and their elders.
Chanda Care Services will provide a support system for families and caregivers of clients and will provide home management care to clients who want to remain as independent in their home for as long as possible. We offer Advocacy packages in addition to services such as Home Making, Companionship, Personal Care, Tuck In Services, 24H Care, Pet and Plant Care, and Resource Linkage.
At Chanda Care, we are trained, professional, and compassionate caregivers who know what it takes to give our highly esteemed clients value for their money. We have a highly organized system that assists in providing an all around care approach.
We can help to ensure you are attending your medical appointments, assist with linking you to specialists, ensure you have the appropriate nursing care, recommend alternative pain management and recommend mental health counselors. We can also guide you in your medication management. We offer a complexity of services that will grow with time and development.
Chanda Care was built on the foundation that every human can make a difference, no matter how big or small. We want to play a part in our communities to provide empathetic care to our aging population. We intend to provide support to clients with any unique need(s). We believe in providing a holistic approach for our clients.
Chanda Care wants to help bridge the gap between the needs of clients and that lack of resources. Our goal is to help maintain their health and wellbeing set by the care team. Our caregivers not only do the tasks set forth, but also have input as their advocate. They build relationships with the clients which helps the case manager navigate the healthcare system. We believe in a 100% team approach.

Chantal Bernard
Certified Case Manager and Registered Nurse with extensive health industry experience since 2003. Chantal became a nurse as she believes nursing has always been her calling. She wants to advocate, educate, and serve her community and purpose. Her passion is to ultimately provide the best care for each individual life she touches.
