Chanda Care Services offers a support system for families and caregivers of clients and provides home management care to clients who want to remain as independent in their home for as long as possible.  We offer Advocacy packages in addition to services such as home making, companionship, personal care, tuck-in services, pet and plant care, and resource linkage.

Steps to Receive Home Health Care in Western MA

Initial Call

Once we've received your information, we will have a short 5-10 minute call to answer any of your questions and concerns and set up a time to meet and review your needs.

In-Home Care Assessment

Here we will answer all your questions, and ask some of our own to craft a Care Plan customized to your unique needs.

Start of Home Care

We will come with our Caregiver on the first day of care to go through the Care Plan together and help with introductions.

Client Visits

Once we have begun care, we will pop in at times our Caregiver is there to see how things are going and determine if we need to adjust the Care Plan.

Certified Case Manager and Registered Nurse with extensive health industry experience since 2003. Chantal became a nurse as she believes nursing has always been her calling. She wants to advocate, educate, and serve her community and purpose. Her passion is to ultimately provide the best care for each individual life she touches.
